Case Manager - intake and assessment (Milwaukee, WI)

Foxglove Willow Housing provides affordable homes and support services around Milwaukee. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident services. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Milwaukee, with some flexibility after the first few months. • We reply to every applicant.
