Case Manager - intake and assessment (Miami, FL)

Stonegate West Care Services Partners is a social-care provider serving older adults and people with disabilities around Miami. We are hiring a Case Manager to take on intake and assessment. We train and promote from within. What you will do • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Miami, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
