Case Manager - intake and assessment (Memphis, TN)

Rivermark Ashton Care Services is looking for a Case Manager. The job centres on intake and assessment for visit scheduling, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish.
