Case Manager - intake and assessment (Madison, WI)

Summit Fox Early Learning Group is looking for a Case Manager. The job centres on intake and assessment for family communication, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Madison. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Madison. • We reply to every applicant.
