Case Manager - intake and assessment (Louisville, KY)

We are growing our team at Bright Union Health Partners and adding a Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for insurance authorisations. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Full-time, Monday to Friday. • On site in Louisville. • Applications are reviewed on a rolling basis.
