Case Manager - intake and assessment (Louisville, KY)

Amberfield Orchard Care Services supports families and individuals in Louisville with personal care and coordination. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of more than 1,000 people based in Louisville. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Louisville. • We will tell you what to expect at each step of the hiring process.
