Case Manager - intake and assessment (Las Vegas, NV)

Lumen Birch Care Services Partners is a social-care provider serving older adults and people with disabilities around Las Vegas. We are hiring a Case Manager to take on intake and assessment. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for licensing. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Las Vegas. • We will tell you what to expect at each step of the hiring process.
