Case Manager - intake and assessment (Knoxville, TN)

Redstone Union Health Group is a healthcare provider in Knoxville with clinics and support teams, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Knoxville. • Applications are reviewed on a rolling basis.
