Case Manager - intake and assessment (Jacksonville, FL)

Summit Analytics is looking for a Case Manager. The job centres on intake and assessment for case files, and it would suit someone who likes steady, practical work. You would join a large organisation of about 839 people based in Jacksonville. What you will do • Help with applications and appointments for case files. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with case files. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Jacksonville, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
