Case Manager - intake and assessment (Jacksonville, FL)

We are growing our team at Bluepeak Cedar Health Group and adding a Case Manager whose main responsibility will be intake and assessment. Clinicians and administrators work side by side. What you will do • Help with applications and appointments for insurance authorisations. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Jacksonville. • We reply to every applicant.
