Case Manager - intake and assessment (Indianapolis, IN)

Clearpath Glen Civic Services Group is looking for a Case Manager. The job centres on intake and assessment for procurement, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Indianapolis. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for procurement. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with procurement. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Indianapolis. • Applications are reviewed on a rolling basis.
