Case Manager - intake and assessment (Indianapolis, IN)

We are growing our team at Lumen Summit Learning Group and adding a Case Manager whose main responsibility will be intake and assessment. We follow the school calendar and we like people who like learning. What you will do • Help with applications and appointments for student accounts. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with student accounts. Working arrangement • Full-time, Monday to Friday. • On site in Indianapolis. • We will tell you what to expect at each step of the hiring process.
