Case Manager - intake and assessment (Honolulu, HI)

Bright Fern Care Services provides in-home and community support in Honolulu. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of more than 1,000 people based in Honolulu. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Honolulu. • We reply to every applicant.
