Case Manager - intake and assessment (Honolulu, HI)

We are growing our team at Stonegate Meridian Civic Services and adding a Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for constituent requests. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Honolulu, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
