Case Manager - intake and assessment (Honolulu, HI)

Lumen Fern Care Services Partners is looking for a Case Manager. The job centres on intake and assessment for family communication, and it would suit someone who likes steady, practical work. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Honolulu, with some flexibility after the first few months. • We reply to every applicant.
