Case Manager - intake and assessment (Hartford, CT)

Cedar Bridge Care Services Group is a social-care provider serving older adults and people with disabilities around Hartford. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for client records. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Hartford. • We will tell you what to expect at each step of the hiring process.
