Case Manager - intake and assessment (Hartford, CT)

Ironwood Mill Care Services is looking for a Case Manager. The job centres on intake and assessment for client records, and it would suit someone who likes steady, practical work. We train and promote from within. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client records. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Full-time, Monday to Friday. • On site in Hartford, with some flexibility after the first few months. • We reply to every applicant.
