Case Manager - intake and assessment (Hartford, CT)

Silverline Ember Housing is a nonprofit housing provider in Hartford. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for lease compliance. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Full-time, on a regular day shift. • On site in Hartford. • Applications are reviewed on a rolling basis.
