Case Manager - intake and assessment (Hartford, CT)

Stonegate Vista Community Partners is a nonprofit in Hartford supporting families with food, housing and job help. We are hiring a Case Manager to take on intake and assessment. We are small enough to know everyone's name. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for volunteer shifts. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Hartford. • We reply to every applicant.
