Case Manager - intake and assessment (Hartford, CT)

We are growing our team at Greenridge Cobalt Early Learning and adding a Case Manager whose main responsibility will be intake and assessment. You would join a small organisation of about 90 people based in Hartford. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for enrolment. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, with a steady weekday schedule. • On site in Hartford, with some flexibility after the first few months. • We reply to every applicant.
