Case Manager - intake and assessment (Grand Rapids, MI)

Redstone West Care Services Group is a social-care provider serving older adults and people with disabilities around Grand Rapids, and we need a Case Manager to lead our intake and assessment. Teams support each other, especially on hard days. What you will do • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish.
