Case Manager - intake and assessment (Fresno, CA)

Silverline Ashton Care Services is a social-care provider serving older adults and people with disabilities around Fresno. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of more than 1,000 people based in Fresno. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Fresno. • We reply to every applicant.
