Case Manager - intake and assessment (Fresno, CA)

Lumen Nest Housing provides affordable homes and support services around Fresno. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for resident services. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Fresno. • We reply to every applicant.
