Case Manager - intake and assessment (Fort Worth, TX)

Greenridge Linden Health Partners is looking for a Case Manager. The job centres on intake and assessment for patient education, and it would suit someone who likes steady, practical work. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient education. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with patient education. • Bilingual English and Spanish.
