Case Manager - intake and assessment (El Paso, TX)

Ironwood Rowan Health Group provides primary and specialty care in El Paso, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 152 people based in El Paso. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in El Paso. • We will tell you what to expect at each step of the hiring process.
