Case Manager - intake and assessment (El Paso, TX)

We are growing our team at Cedar Basin Health and adding a Case Manager whose main responsibility will be intake and assessment. We are busy and kind, and we look after each other. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, with a steady weekday schedule. • On site in El Paso, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
