Case Manager - intake and assessment (Detroit, MI)

Lumen West Care Services is looking for a Case Manager. The job centres on intake and assessment for care plans, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for care plans. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Detroit, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
