Case Manager - intake and assessment (Detroit, MI)

Clearpath Wren Care Services supports families and individuals in Detroit with personal care and coordination. We are hiring a Case Manager to take on intake and assessment. We train and promote from within. What you will do • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Full-time, on a regular day shift. • On site in Detroit. • We reply to every applicant.
