Case Manager - intake and assessment (Des Moines, IA)

We are growing our team at Northwind Union Health and adding a Case Manager whose main responsibility will be intake and assessment. Continuing education is supported. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Full-time, Monday to Friday. • On site in Des Moines. • Applications are reviewed on a rolling basis.
