Case Manager - intake and assessment (Des Moines, IA)

Bluepeak Linden Health is a healthcare provider in Des Moines with clinics and support teams, and we need a Case Manager to lead our intake and assessment. We are busy and kind, and we look after each other. What you will do • Help with applications and appointments for electronic records. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Des Moines. • We will tell you what to expect at each step of the hiring process.
