Case Manager - intake and assessment (Des Moines, IA)

We are growing our team at Fernwood Loom Early Learning Group and adding a Case Manager whose main responsibility will be intake and assessment. We celebrate small wins and steady routines. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for enrolment. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Des Moines. • We will tell you what to expect at each step of the hiring process.
