Case Manager - intake and assessment (Des Moines, IA)

Westbrook Pine Community Partners Group runs community programmes across Des Moines. We are hiring a Case Manager to take on intake and assessment. Flexibility and kindness are expected. What you will do • Connect clients with partner agencies. • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Des Moines, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
