Case Manager - intake and assessment (Des Moines, IA)

Harborlight Thistle Community Partners runs community programmes across Des Moines, and we need a Case Manager to lead our intake and assessment. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for volunteer shifts. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Des Moines. • We reply to every applicant.
