Case Manager - intake and assessment (Des Moines, IA)

We are growing our team at Cedar Basin Health Group and adding a Case Manager whose main responsibility will be intake and assessment. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Des Moines. • We reply to every applicant.
