Case Manager - intake and assessment (Denver, CO)

Summit Fox Community Partners runs community programmes across Denver, and we need a Case Manager to lead our intake and assessment. You would join a large organisation of about 241 people based in Denver. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Denver, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
