Case Manager - intake and assessment (Cleveland, OH)

Clearpath Basin Civic Services Partners works with county and city offices around Cleveland, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 16 people based in Cleveland. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for public records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Cleveland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
