Case Manager - intake and assessment (Cleveland, OH)

Northwind Clear Care Services supports families and individuals in Cleveland with personal care and coordination, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Cleveland. • Applications are reviewed on a rolling basis.
