Case Manager - intake and assessment (Cleveland, OH)

Rivermark Vale Housing is looking for a Case Manager. The job centres on intake and assessment for resident services, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident services. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with resident services. • Bilingual English and Spanish.
