Case Manager - intake and assessment (Chicago, IL)

Harborlight Field Community Partners Group is a Chicago charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Chicago, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
