Case Manager - intake and assessment (Chicago, IL)

Silverline Vista Community Partners runs community programmes across Chicago. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of more than 1,000 people based in Chicago. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for benefits enrolment. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Full-time, Monday to Friday. • On site in Chicago, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
