Case Manager - intake and assessment (Chicago, IL)

We are growing our team at Ironwood Oak Health Group and adding a Case Manager whose main responsibility will be intake and assessment. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Chicago, with some flexibility after the first few months. • We reply to every applicant.
