Case Manager - intake and assessment (Chicago, IL)

We are growing our team at Greenridge Bright Community Partners and adding a Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Chicago. • We will tell you what to expect at each step of the hiring process.
