Case Manager - intake and assessment (Buffalo, NY)

Ironwood Sparrow Health provides primary and specialty care in Buffalo, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for clinic supplies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Buffalo. • We reply to every applicant.
