Case Manager - intake and assessment (Buffalo, NY)

We are growing our team at Foxglove Kestrel Housing and adding a Case Manager whose main responsibility will be intake and assessment. You would join a large organisation of more than 1,000 people based in Buffalo. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for repair requests. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with repair requests. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Buffalo, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
