Case Manager - intake and assessment (Buffalo, NY)

Lumen Housing Group is a nonprofit housing provider in Buffalo. We are hiring a Case Manager to take on intake and assessment. You would join a small organisation of about 39 people based in Buffalo. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for waiting lists. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Buffalo, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
