Case Manager - intake and assessment (Boston, MA)

Lumen Thistle Early Learning Group is looking for a Case Manager. The job centres on intake and assessment for family communication, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Boston, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
