Case Manager - intake and assessment (Birmingham, AL)

Rivermark Mill Care Services Group is a social-care provider serving older adults and people with disabilities around Birmingham. We are hiring a Case Manager to take on intake and assessment. The work is personal and so is the culture. What you will do • Help with applications and appointments for licensing. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Birmingham. • Applications are reviewed on a rolling basis.
