Case Manager - intake and assessment (Birmingham, AL)

Rivermark Mill Care Services Group is looking for a Case Manager. The job centres on intake and assessment for care plans, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for care plans. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Birmingham. • We reply to every applicant.
