Case Manager - intake and assessment (Baltimore, MD)

Northwind Vista Housing Group is a nonprofit housing provider in Baltimore, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for waiting lists. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Baltimore. • Applications are reviewed on a rolling basis.
