Case Manager - intake and assessment (Baltimore, MD)

Clearpath Junction Community Partners is looking for a Case Manager. The job centres on intake and assessment for donor records, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for donor records. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Baltimore. • Applications are reviewed on a rolling basis.
